a Civil Liberties Complaint

Capital Region Chapter. To help us assist you we are providing a
complaint form which gives us information to better assess your
complaint. You may download
the form in Word format, enter your complaint and return the form
to us. If you are unable to edit a Word document, please copy and
paste the sample of the form below into an alter text editing program
or print this page and fill it out by hand. Return the completed

not your legal representative in any way. Also keep in mind that
not every problem has a legal solution or is a problem that the

you receive from us is of a general nature; you should consult your
own attorney regarding any specific concerns, such as meeting any
relevant deadlines. After we receive the completed complaint form,
we will respond to you either by phone or mail as soon as possible.
